This programme has been developed to support analysts and other health and care professionals working in population health to build their knowledge and skills within this area. Population health management (PHM) is central to the development of and is a new focus of analytical work for analysts working across these partnerships.
Population health management (PHM) is an approach that uses data to help health and care systems to improve population health and wellbeing. The Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM, with primary and secondary care providers needing to risk stratify and proactively support key population groups with personalised care models. PHM will become increasingly important as systems move to formally establish ICSs and will be pivotal to the way that the new systems will work together to improve the health of their populations.
The elearning modules that have been developed are:
Introduction to Population Health Management
An Introduction to Population Health Management – what it is, why it’s important and what you need to get started
This session describes the main stages in the process to assess needs of a population
A Practical Introduction to Population Segmentation
A practical step by step run through of the methods needed to undertake population segmentation as part of a population health management approach, with signposting to complementary resources, examples and user stories
A practical run through of the methods needed to undertake risk stratification as part of a population health management approach
Introduction to Population Health Surveillance
This session describes the concept of population health surveillance and how it can be used to identify and monitor threats to the health of the population, so that actions can be taken.
This session describes the principles of good data visualisation for charts and maps in order to produce effective intelligence.
Knowledge to action is the process of landing your resources correctly to the required user at the appropriate time. Explore knowledge to action and the “K2A Essentials” toolkit to understand the value of this skill.
The hospital episode statistics data set provides valuable hospital activity data. This module supports new users in understand what is in the data set, how to understand the data and how to extract the data.
On completion of these modules, health and care professionals will be able to describe the concepts of population health management and how it fits with current health policy. They will understand a variety of skills that are essential to anyone undertaking population health analysis; they will discover the hospital episodes statistics data set; understand methods such as health needs assessment, and segmentation and risk stratification; learn the principles of good data visualisation and surveillance. They will also understand how to communicate their intelligence effectively to the appropriate stakeholders.
